Orthopedic and Sports Rehabilitation

Horizon Rehabilitation Centers offer comprehensive orthopedic physical therapy services that address musculoskeletal impairments and functional limitations. Our physical therapists provide research-based care to individuals, with an emphasis on helping the patient achieve his or her stated goals.  Specialized care is provided in a variety of areas which include total shoulder, hip, and knee replacements; shoulder injuries/surgeries; and hip and knee injuries/surgeries.  At Horizon Rehabilitation Centers, your overall outcome and return to function is important to us.

Frozen Shoulder Rehabilitation

The frozen shoulder or adhesive capsulitis program was developed utilizing the Niel Asher technique for frozen shoulder.  Although this program was developed for treating frozen shoulder, it has proven extremely successful for treating nearly all shoulder pathologies.  These include tendonitis (biceps, supraspinatus and rotator cuff) and bursitis (subacromial and deltoid), rehabilitation after surgery, manipulation under anesthesia and osteoarthritis.

This technique has undergone several clinical trials and has shown significant improvements over traditional physical therapy for treating frozen shoulder.  This program requires fewer visits and is less painful than traditional therapy for this condition.  Please see www.frozenshoulder.com for more information regarding this program.

Post Concussion Testing

Concussions are brain injuries that require specialized care to achieve the greatest recovery possible.  At Horizon Rehabilitation Centers, we work closely with physicians to ensure that each concussion is monitored until symptoms have resolved and return to play is appropriate.  Our team of physical therapists is trained to identify signs and symptoms of concussion, interpret computerized concussion test results and administer graded return to play exercise programs.  Additionally, our therapists can provide vestibular rehabilitation or treatment for cervical spine injuries that can occur when a concussion is sustained.

ACL Tear Prevention Program

ACL injuries are very common in high school sports where jumping and fast changes of direction occur.  While it is impossible to completely eliminate the possibility of an ACL injury, there are ways to decrease the likelihood of a young athlete sustaining an ACL injury.  Our team of physical therapists are trained to identify specific deficiencies in an athlete that could place them at risk for an ACL injury.  We then structure an individualized treatment plan to treat weaknesses and help reduce their risk for ACL injury.

Hip Osteoathritis (OA) Prevention and Management Program

Hip osteoarthritis can significantly impact a patient’s life, with functional mobility loss and pain limiting their participation in life activities.  The OA prevention and management program at Horizon Rehabilitation Centers follow the evidence-based recommendations from the American College of Rheumatology and incorporates a multiple-method approach to improve function and decrease pain.  This approach includes

Joint mobilization – recent research has shown that this hands-on approach can significantly improve tissue mobility and decrease pain.  The manual physical therapists at Horizon Rehabilitation Centers are skilled at delivering these interventions.

  • Therapeutic exercise – with pain, musculature around the hip may weaken, which may lead to further pain, disability, and loss of functional mobility.  At Horizon Rehabilitation Centers, the exercises are specific and focused on patient deficits observed during the initial evaluation, and observations made at subsequent visits.  At Horizon Rehabilitation Centers, all treatments are provided by physical therapists, allowing for ongoing evaluation and modification of interventions.
  •  Balance and Proprioceptive Training – with an increase in hip pain and decreased mobility, a patient’s balance and proprioceptive capabilities may also become impaired, leading to increased mobility difficulties.  At Horizon Rehabilitation Centers, the physical therapists include targeted balance and proprioceptive interventions to maximize functional mobility improvement.
  • Our belief at Horizon Rehabilitation Centers is that a multiple-intervention approach, carried out on a one-on-one basis with a physical therapist, leads to maximum improvement of a patient’s functional mobility while reducing their pain.

Custom Orthotic Fabrication

Custom orthotics can be used to help correct over thirty common diagnoses related to poor foot function, including pathology in the hip, knee, and foot. Because foot function affects the entire alignment of bones leading to the spine, pain and dysfunction in the knee, hip, and low back often result if foot position is poor. Custom-fit orthotics are fabricated to your foot, allowing the orthotic to restore normal foot positioning with greater effect than a common off-the-shelf orthotic. Restoring normal foot function aids your body in healing itself and often improves the effectiveness of other necessary treatment or therapies.
